Click here for a list of pre-approved community service activities.  If you select something from this list, no pre-approval is needed.  Simply fill out the participation form and have your supervising adult sign off on your hours.
Click here for the Community Service Participation Form.  This form must be filled out for each community service activity completed.  This is the form that is signed by your supervising adult of the community service activity.
